           Thank you to all who attended the Little Rivers Health Care Legislative Town Hall on March 30 in Fairlee. We were grateful to be joined by Vermont State Representative Jim Masland, Rebecca Holcombe, Mike Tagliavia, Monique Priestley, and Vermont State Senator John Benson for a thoughtful discussion on issues impacting our community.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;

           Advocating for Food Is Medicine at the Capitol

           At this year’s Vermont Capital Advocacy Day, Little Rivers Health Care proudly raised its voice to champion a simple but powerful idea: food is medicine.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Emilie Perry, LRHC’s Food Is Medicine Program Coordinator, shared the story of how Maureen Boardman, NP and Director of Clinical Quality made a small, patient-centered act by filling her car with fresh produce for patients, which grew into a comprehensive, community-driven Food Farmacy program. Today, that program serves rural communities across Bradford, East Corinth, Newbury, Wells River, and neighboring New Hampshire towns, addressing one of the most pressing health challenges we face: the connection between nutrition and chronic disease.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Nearly one in five LRHC patients lives with conditions like heart disease, hypertension, or type 2 diabetes; conditions deeply influenced by access to healthy food. Through expanded screening efforts, we now identify food insecurity in real time and connect patients to critical resources like WIC, SNAP, and local food supports. For those with chronic conditions, our Food Farmacy provides fresh produce, eggs, and other staples every three weeks, totaling nearly 1,000 pounds of food distributed weekly.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           This work goes beyond food access. It is about building relationships, offering education, and empowering patients to take control of their health. From recipes and kitchen tools to partnerships with local farms and healthcare organizations, LRHC is creating a model of care that integrates nutrition directly into clinical practice.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           The results speak for themselves. Patients enrolled in our programs are experiencing measurable improvements in blood pressure and cholesterol, fewer missed appointments, and reduced stress. Many report eating more fruits and vegetables and cooking more meals at home—small shifts that lead to lasting health outcomes.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           As Emilie emphasized in her remarks, while grant funding has allowed this work to grow, it is not a sustainable long-term solution. Advocacy efforts like Capital Day are critical to advancing policies that recognize and reimburse Food Is Medicine programs as essential healthcare services.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           LRHC is committed not only to continuing this work, but to building a replicable model that can support other rural healthcare systems. With continued collaboration and policy support, we can ensure that every patient has access to the nutrition they need to thrive.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;

      <title>Televisits: Remote Visits With Your Provider</title>

           Televisit is not a replacement of traditional medicine; it is an extension of our traditional model of care. The Televisit allows our Doctors or Nurse Practitioners to diagnose and treat patients who are unable to make it to the office. From your computer or with the Healow app and a compatible mobile device, you can have a face-to-face conversation with a Doctor or Nurse Practitioner from the convenience of your home, hotel, or where ever you may be traveling.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      &lt;span&gt;&#xD;
        
            You must be web enabled on the Patient Portal to schedule a Televisit. If you are not, please ask our receptionist to web enable your account. 
           &#xD;
      &lt;/span&gt;&#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
&lt;/div&gt;&#xD;
&lt;div data-rss-type="text"&gt;&#xD;

           How It Works
          &#xD;

    &lt;span&gt;&#xD;
      
           Call our office to schedule an appointment for a Televisit. After your appointment is scheduled you will receive an email confirmation with a link. You can either use the link provided in your email or the Healow app on your mobile device. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           Ten minutes before your scheduled appointment, click on the link in your email or open the Healow app on your mobile device, and fill out a brief questionnaire about your visit. You will then be connected to a doctor or nurse practitioner via 2-way video from your computer or smartphone. Your doctor or nurse practitioner will diagnose your condition, answer your questions, and, if needed, send a prescription to your preferred pharmacy.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           *Prescription availability is dependent on the doctor’s or nurse practitioner’s judgment and, if out of state, may be restricted by state regulations.
          &#xD;
    &lt;/span&gt;&#xD;

           Treatment

           Remote visits are a good way to get treatment for many minor medical conditions, including: acne, bites or stings, birth control pill consultations, cough, congestion, runny nose, diarrhea, nausea, vomiting, fever, headaches, hives, pink eye, lice, skin infections, sprains and strains, travel medications, and much more.
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/p&gt;&#xD;
  &lt;p&gt;&#xD;
    &lt;span&gt;&#xD;
      
           If you have a medical emergency, please dial 911 immediately and do not attempt to receive treatment via a remote visit. 
          &#xD;
    &lt;/span&gt;&#xD;
  &lt;/p&gt;&#xD;

           Policies for Televisit appointments are the same as in-office appointments:

            When making a Televisit, appointment fees are due at the time the appointment is scheduled.

            10 min prior to your appointment time click on the email link you have been sent or use the healow mobile app and your mobile device to start your Televisit.

      &lt;span&gt;&#xD;
        
            If you need to cancel or reschedule your appointment, please call the office at least 2 hours ahead of appointment time to avoid any no-show fees.

      &lt;span&gt;&#xD;
        
            Logging onto your Televisit is the same as signing-in when in the office.

      <title>Everyone Eats Vermont - Little Rivers Health Care Spotlight</title>

         Little Rivers Health Care partnered with Capstone Community Action to distribute meals to folks experiencing barriers to food access as part of the Everyone Eats Vermont program. Starting in August 2020 and continuing through today, LRHC care coordinators Pam Spear and Sasha Emerson, along with other staff and volunteers, have been distributing 75 meals every single week to 15-20 families and individuals. The meals have gone out regardless of weather or holidays. The Everyone Eats Vermont program is a testimony to a community that cares about their neighbors in the most challenging of times.

           The staff listed above have recently completed the AgriSafe Network Nurse Scholar Program. They join other nurses across the nation who have invested in their professional training and their communities. Our clinics are located in 3 locations throughout the Upper Valley of Vermont: Bradford, East Corinth, and Wells River.

           AgriSafe Nurse Scholar is an 18-hour educational series which targets agricultural health and safety issued, risks, and prevention measures that can impact the short and long-term wellness of community families and businesses that depend on production agriculture. Topics include ergonomics, women’s health, youth in the workforce, mental health, hearing and respiratory protection, animal borne diseases, pesticide and chemical safety, and several emerging issues.

           Ashley Murray serves at Little Rivers Health Care as an LNA and MA. She is a graduate of Southern Vermont College and is currently Enrolled at Community College of Vermont. She has worked for LRHC for 5 years. Her background has included being an LNA for St Johnsbury Health and Rehab and Rowen Court. When asked about the AgriSafe program Ashley said, “I spent 18+ hours studying and learning about conditions and struggles that farmers and their families face. This was a huge eye opener and only increased my appreciation for all farmers and families, but especially our locals! I look forward to being able to put this education to use!”

           Liz Henson serves at Little Rivers Health Care as an LNA. She is a Half time student at White Mountain Community College in Littleton working towards a nursing degree. She has been an LNA for over 20 years. She has worked for LRHC for just over a year. She received her LNA license at Glencliff in 1999.  She has worked as a patient care technician at the Children’s Hospital at Dartmouth and in the pediatric ICU. Later she worked at Cottage Hospital as a tech in the Emergency Room. When asked about the AgriSafe program Liz said, “the AgriSafe nurse scholar program was packed with valuable information about rural healthcare and some of the challenges that may be faced when working with the agricultural population.”

          Vermont organized COVID-19 testing sites for asymptomatic people to sign up for a test. Upcoming testing sites adjacent to our greater region that still have openings are on Tuesday 5/19 in Barre and Saturday 5/23 in St. Johnsbury. Sign up is required.           Through yesterday, we performed 200 tests on Little Rivers patients. One test was positive as reported on April 16, and we are waiting on results of only three tests. We performed 25 tests from outside referrals with no positive results as of yesterday.

  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;

          VDH recommends continuing with health and safety precautions including 6-ft distancing, wearing a face mask, washing hands frequently, and staying home if you are sick or have been exposed to someone who has COVID-19.

  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;
    
          VDH recommends keeping your social circle small, staying close to home, and limiting non-essential travel.

  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;
    
          People age 65 and older and people with underlying medical conditions are more likely to develop more severe symptoms, should continue to stay at home, and should connect with others by phone, computer, or video.

  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;
    
          Little Rivers serves many communities in Eastern Orange County Vermont and surrounding areas, and this region has many people of age 65 and older and many with underlying health conditions. Please follow the stay at home guideline and encourage those you know in this group to act similarly. Little Rivers Care Coordinators, Chronic Care Nurses, Behavioral Health Providers, and Family Medicine Practitioners &amp;amp; Nurses are available to see patients by televisit and phone. Care Coordinators and Chronic Care nurses commonly call to check in with patients. We encourage you to call and to do the same with those close to you.           On the New Hampshire side of the river, there have been 3,160 cases and 133 deaths attributed to COVID-19. On May 6, NH announced 19 new deaths (the highest single day increase in the number of deaths). On May 1, Governor Sununu announced a new stay at home order in effect until May 31.

Windsor County has 45 cases, Washington County has 38 cases, Caledonia County remains at 14 cases and Orange County at 8 cases. Windsor has 2 deaths so far, Washington 1, and none in Caledonia and Orange Counties. Orange County Vermont remains at 2 cases per 10,000 people. In our region, there are confirmed cases in Lyndon, Kirby, St. Johnsbury, Waterford, Marshfield, Groton, Orange and Fairlee.

          Dr. Genereaux delivered a baby this weekend!

          Last week we had a total of 469 patient visits including 51 televisits and 225 phone visits. Our overall visit count is about 60 to 65% of our pre-pandemic visit rate, and behavioral health visits are at the same rate as the pre-pandemic rate. Our East Corinth Clinic continues in-person care tomorrow and again on Friday. Due to a decrease in patients with symptoms of  respiratory illness, the Bradford clinic is decreasing the respiratory care only hours to 2:30 to 5PM daily (and as a result increasing customary appointment hours).  Please call first, and we will coordinate a visit for you in a safe manner and regardless of your ability to pay. If you need a primary care provider, we will happily help you establish at Little Rivers. 
         &#xD;

  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;

          We are strategically planning for an increase of in-person visits and for warmer weather. We are (1) considering alternating provider appointments of in-person and televisit to minimize waiting room occupancy, (2) researching patient pager systems to allow patients to wait for appointments outside, (3) setting up more spaces outside for patients to sit, and (4) preparing for pre-visit planning calls to remind patients to come along when possible and to review instructions for arrival at the clinic. We are researching special filters and air treatment devices for our air handling systems and air conditioning units to prepare for warmer weather. We look forward to sharing our plans for continued safe access to health care.

          New Hampshire Governor Chris Sunnunu communicated today that dental offices in New Hampshire will be able to reopen as early as Monday under new guidance from the state. New Hampshire Health Commissioner Lori Shibinette said that in terms of numbers, the potential exposure at a dental office is lower than at a long term care facility. 
         &#xD;

  &lt;div&gt;&#xD;
    &lt;br/&gt;&#xD;
  &lt;/div&gt;&#xD;
  &lt;div&gt;&#xD;
    
          The East Corinth clinic operated a second day this week today with in-person visits. About ten patients were seen in-clinic this week. Next week will have the same schedule of virtual operations through the week and in-clinic appointments on Tuesday and Friday.

    &lt;br/&gt;&#xD;

          Upcoming Meetings: Join us on Monday at 10:00 AM for our “No Barriers to Care” meeting about resources to access health care and prescription drugs through the support of our care coordinators and financial resources available through Little Rivers for access to care regardless of ability to pay. We will review navigation of Vermont Health Connect that has open enrollment through May 15th, sliding fee scale, and our 340 discount prescription drug program.

          Vermont launched an online compliance tool to report violations of the COVID-19 Executive Order. The tool accepts complaints of non-compliant lodging properties and online reservation suspension, non-compliant business operation, and non-compliance of restaurant on-premise consumption of food and drink. Recovered includes people who have tested positive, report that they have recovered, and for which 30 days have passed since the person’s illness began. The total recovered count for today is 718.

          Little Rivers behavioral health providers will begin seeing patients in-clinic starting on May 18th. Each provider will be in-clinic at least one full day per week to start and all will continue with Televisits and phone visits. Yesterday was the first day that the East Corinth Clinic resumed delivery of  in-person, in-clinic health services. Six patients had in-person visits in addition to Televisits and phone visits. The East Corinth team is eager and ready to safely care for patients in clinic on Tuesdays and Fridays.

          The Little Rivers team of medical providers had a monthly meeting early this morning. The meeting was conducted by conference call and many topics were reviewed regarding the pandemic. Planning is under way to start up in-person care for well child visits and with special attention to  pediatric vaccinations , especially for children under 2 years old. Well child visits may be provided with a well check by televisit or by phone and a follow up in-person 5 to 10-minute physical. Initial planning was discussed for time-sensitive adult annual exams. The team will continue to plan to open up more appointments in a cautious manner and in accordance with CDC and State based guidelines, and we will share more information just as soon as it is available. The provider team also discussed possible preliminary stages for resuming medical student preceptorships. Many of our providers work with students from the Geisel School of Medicine at Dartmouth and the Robert Larner College of Medicine at UVM. Students spend a portion of their time observing our primary care physicians in their offices.

          Governor Phil Scott announced last week that the state is working to significantly increase and strengthen its testing and contact tracing programs in order to quickly identify, contain and suppress cases and outbreaks of COVID-19. Vermont is now urging medical providers to refer patients with mild symptoms for testing and to test symptomatic children (those younger than 18 years). Patients should be referred to hospitals, associated test centers, and centralized testing sites such as local Federally Qualified Health Centers. Little Rivers conducts tests for Little Rivers patients in Wells River and Bradford, and we also serve as a centralized testing center in Wells River for patients referred from other medical providers (all patients regardless of referral source).

          Last week we had a total of 442 patient visits including 56 televisits and 234 phone visits. Our overall visit count is about 60 to 65% of our pre-pandemic visit rate, and behavioral health visits are at the same rate as the pre-pandemic rate. The National Association of Community Health Centers reported today that community health centers in rural areas will receive much needed funds. Community health centers have suffered plummeting revenues because patients are avoiding routine primary care during the pandemic. Health centers serve 1 in 5 rural residents and operate with slim margins. Little Rivers has worked hard to provide COVID-19 screening, testing, and safe primary care when needed in our clinics and through televisits &amp;amp; phone visits whenever possible. Our East Corinth Clinic resumes in-person care starting tomorrow on Tuesdays and Fridays. Please call first, and we will coordinate a visit for you in a safe manner and regardless of your ability to pay.           Vermont Hospitals can now resume nonessential outpatient clinic visits, diagnostic imaging and outpatient surgeries and procedures, according to a statement from Vermont Governor Scott's office. Surgeries that require overnight stays are still on hold. Scott said Vermonters should expect the coronavirus state of emergency to extend beyond the May 15 expiration of his current “stay home, stay safe” mandate.
