Our mission is to provide respectful, comprehensive primary health care for all residents in our region, regardless of their ability to pay. We offer quality health care services to everyone. In the spirit of community, we make efforts to reach out and welcome those who need health services, but may have insufficient means to access them. We commit ourselves to continually reduce the burden of illness, injury, and disability, and to improve the health and quality of life of those for whom we care.

Do you avoid getting regular health care? Maybe your work schedule doesn't allow you to come during regular hours? Maybe you think you can't afford it? Maybe the staff at your doctor's office isn't very friendy or welcoming?

At LRHC, we want to be as responsive to your needs as possible. We strive to make health care as convenient as we can, and we plan to be offering evening and weekend hours by early winter, 2015. We offer a sliding fee scale for those who are uninsured and under-insured, (see below) and our care coordinators will always help people sign up for any of the Vermont Health Connect insurance programs. We pride ourselves on having staff and clinicians who are kind, compassionate, and treat you with respect and dignity.

We are accepting new patients, and welcome you to become a patient at LRHC - where you will also be a full member of your healthcare team. Completion and submission of forms in our new patient packet is the first step towards establishing care at Little Rivers. Forms are available from the links listed below, or at any of our clinics as listed below, or by calling our new patient coordinator, Maegan, at 802-222-3025


Completed packets may be submitted at any of our clinics, or faxed to 866-939-1476. After packets are received, our new patient coordinator reviews them. If any required information is missing, she will reach out to you. The most important requirement is the receipt of completed medical release forms. Our coordinator next reaches out to your care providers for medical records. The last step includes the assignment of a Little Rivers provider as your new primary care provider. The process often takes from three to six weeks and ends with your first office visit.
