At LRHC, we believe that everyone deserves accessible, affordable, and compassionate care. That’s why we offer a sliding fee scale for uninsured or underinsured patients and have dedicated care coordinators who can help you explore coverage options through Vermont Health Connect.

Ready to Join the LRHC Community?
Getting started is simple:
Get a New Patient Packet:
- Download the PDF packet or fill it out online.
- Pick one up at any LRHC clinic
- Or call Maegan, our New Patient Coordinator, at 802-222-3025 to request one
Submit Your Completed Packet:
- Through our online form
- Drop it off at any LRHC location
- Or fax it to Maegan at 866-939-1476
Once we receive your forms, Maegan will review them and follow up if anything is missing. We’ll then request your past medical records and match you with a primary care provider. The full process typically takes 3 to 6 weeks and ends with your first visit.
