Upper Valley Unified Community Collaborative (UVUCC)

The UVUCC is a collaboration of local health and human service organizations who meet virtually once a month to discuss the current health needs of our community, what each organization is doing to support those health needs, where services overlap between organizations, and what resources, skills and technologies could support our efforts. 


Misson

The UVUCC Mission is to improve the quality of life of the people in the communities we serve.


The UVUCC publishes a Community Health Needs Assessment every 3-5 years. The CHNA is a process that identifies and analyzes a community's health needs. It is a way for communities to prioritize health needs, plan how to address unmet health needs, identify gaps in services, strengthen relationships with community partners, and share information with the public.

  • Support and Services at Home (SASH)
  • Tri Valley Transit
  • Capstone Community Action
  • Greater Upper Valley Integrated Services Team and The Public Health Council (GUVIST/PHC)
